Becoming a Patient at Saint Vincent Clinic
Complete the Steps below to become a patient at Saint Vincent Clinic. We currently accept Medicare/Medicaid, some insurance plans, and those who do not have insurance. Complete process is provided by the Patient Care Coordinator.
Step 1
Contact our Patient Care Coordinators to set up an appointment to process registration.

Step 2
Gather as many of the following documents as possible:
Photo ID.
Proof of residency (utility bill or mail with your name, address on it).
Proof of income
Paycheck stub (if applicable)
Last year’s taxes (if applicable)
Step 3
Start with what you have!
Bring in your documents that you can locate. If you’re currently not working or you did not file last year’s taxes, we can help.
Bring what you have, and we’ll help you complete the rest.
FEE SCHEDULE
We use a sliding fee schedule to determine payments. Please contact the front desk for current sliding fee schedule and payment options.
We use the most current Federal Poverty Guidelines to determine financial eligibility.
